La Región “Urabá Antioqueño-Cuencas altas de los ríos Sinú y San Jorge-Bajo Cauca Antioqueño”: “guarida” del paludismo colombiano

In the zone formed by Urabá Antioqueño-Cuencas altas de los ríos Sinú and San Jorge-Bajo Cauca Antioqueño (hereinafter, the Region), 60% of the Colombian cases of malaria are described. The Region is not specifically addressed in official reports and even malaria researchers do not refer to it precisely. The objective of this reviewis to argumentatively describe what is the Region. This is a theoretical descriptive study, which proposes that  the Region should be conformed by 25 municipalities, 21 that are usually included in Bajo Cauca and four additional municipalities of Antioquia (Valdivia, Anorí, Remedios and Segovia), as they have the same ecological and epidemiological conditions and similar levels of malaria endemia. The “new” Region is integrated by 11 municipalities in the Urabá Antioqueño, 4 in the south of Cordoba, and 10 in the Bajo Cauca Antioqueño. The “new” Region has 34,848 km2 and 1,339,297 inhabitants (2015). Furthermore, the Region has large plains and at least four levels of rainfall intensity. Transmission is stable, with no marked fluctuations in the number of malaria cases during the year. Since 1950-1959, the Region has exhibited a high malaria incidence (annual parasitic index >25 per 1,000 exposed) and predominance of P. vivax (60-70% of the total). There 19 species of Anopheles in the Region. It is concluded that there are abundant and solid ecological and epidemiological arguments to propose that the Region would include the 25 municipalities mentioned before, and that the Colombian health authorities should provide periodic information about it.En la zona formada por el Urabá Antioqueño-Cuencas altas de los ríos Sinú y San Jorge-Bajo Cauca Antioqueño (en adelante, la Región) se reportan el 60% del paludismo en Colombia. La Región no es tratada de manera específica en informes oficiales y ni siquiera los investigadores de malaria la tratan en forma precisa. Esta revisióntiene como objetivo describir en forma argumentada qué es La Región. Este es un estudio teórico descriptivo en el que se propone que la Región debe conformarse por 25 municipios, los 21 que usualmente se incluyen en el Bajo Cauca más cuatro de Antioquia (Valdivia, Anorí, Remedios y Segovia), porque tienen las mismas condiciones ecoepidemiológicas y nivel similar de endemia. La Región “nueva” la integran 11 municipios en Urabá Antioqueño, cuatro en sur de Córdoba, y 10 en Bajo Cauca Antioqueño. La Región “nueva” tiene 34.848 km2 y 1.339.297 habitantes (2015). Asimismo, cuenta con grandes planicies y al menos cuatro niveles de intensidad pluviométrica. La transmisión es estable, sin marcadas fluctuaciones en casos anuales de malaria. Desde 1950-1959, la Región tiene alta incidencia palúdica (índice parasitario anual >25 por 1.000 expuestos) y predominio de P. vivax (60-70% del total). Las especies anofelinas halladas en la Región son 19. Se concluye que hay argumentos ecológicos y epidemiológicos abundantes y sólidos para proponer que la Región se integre por los 25 municipios referidos y que las autoridades sanitarias colombianas entreguen información periódica sobre ella
